The survey is still open and everyone with dementia is invited to join in and share! Please participate and share your experiences living with dementia.
Everyone who contributes will be providing valuable information that could raise awareness about dementia. The hope is that with your input, we can help change misconceptions, reduce stereotypes and provide a light that can cut through the shadows of ignorance about dementia.
Individual names will remain confidential.
Survey responses will be used to report compiled results of our collective experiences.
The survey should take about 10 to 15 minutes.
The survey is easier to take on a laptop or tablet instead of a phone.
If you take the survey on your laptop and need to step away, simply leave the window open and start where you left off when you come back to finish the survey.
If you are logged into a google account, a partially completed survey will actually be saved so you can step away and come back to finish later, even if the window closed.
Time to complete the survey will depend on whether you respond to the “feel free to share more” sections, and considering how much you choose to share, you may need a bit more time to complete the survey.

Since 2014, Dementia Alliance International (DAI) has worked tirelessly to support people living with dementia while raising awareness, eradicating stigma and improving quality of life.
The data gathered with this survey will assist us in informing others about dementia and why supporting people with dementia is so important. The data will also aid us as we seek funding to help DAI continue as an organization.
