Since 2014, Dementia Alliance International (DAI) has worked tirelessly to support people living with dementia while raising awareness, eradicating stigma and improving quality of life.
Now, DAI has developed a survey to provide an opportunity for people living with dementia to tell their stories by responding to questions about their experiences living with dementia.
Anyone with a dementia diagnosis is invited to take the survey. Please take the opportunity to share your experiences living with dementia by completing this survey.
Everyone who contributes will be providing valuable information that could raise awareness about dementia. The hope is that with your input, we can help change misconceptions, reduce stereotypes and provide a light that can cut through the shadows of ignorance about dementia.

Survey responses will be used to report compiled results of our collective experiences.
The survey should take about 10 to 15 minutes.
Time to complete the survey will depend on whether you respond to the “feel free to share more” sections, and considering how much you choose to share, you may need a bit more time to complete the survey.

The data gathered with this survey will assist us in informing others about dementia and why supporting people with dementia is so important. The data will also aid us as we seek funding to help DAI continue as an organization.
